The five elements auditors actually check
- Time in / time out (or total minutes) — not “30-minute session” boilerplate, the actual session length delivered.
- Service link — what was targeted, tied to an IEP goal. “Worked on speech” fails; “produced /r/ in initial-position words, IEP goal 2” passes.
- Data — a number. Accuracy, trial counts, cue levels. Notes without measurable response are the most-flagged category in state audit reports.
- Student response / progress statement — one sentence comparing today to the trend.
- Signature and credentials — every note, every time, with your license/credential line.
Write the skeleton during the session, not after
